Which sizes you actually need
32x32 covers browser tabs, 180x180 covers Apple touch icons, and 192x192 plus 512x512 cover Android and progressive web app installs. Supplying those four is enough for every current platform.
Design for 16 pixels, not 512
Your favicon will be seen at tab size almost every time. Detailed logos turn to mud. Use one or two bold characters, or a single simple mark, with strong contrast against both light and dark backgrounds.

Rarely. Modern browsers read PNG icons from link tags. An .ico file is only needed for very old browser support.
Browsers cache favicons aggressively. Do a hard refresh, or add a version query string to the file path.
